[ADMIN] Get failed lead jobs across all orgs.

Uses GSI2 (PK=FAILED_LEAD_JOBS) to fetch all failed jobs in a single
paginated query instead of iterating orgs × failure reasons.
Resolves org names from a single get_all_organizations() call instead
of N individual get_org_record() calls.
