MCP Streamable-HTTP JSON-RPC endpoint, dual-era: 2026-07-28 requests (per-
request ``_meta``) are served statelessly; legacy clients use ``initialize``
and may batch. ``initialize`` / ``server/discover`` / ``tools/list`` need no
key; tool calls authenticate with the ``X-API-Key`` header, an OAuth access
token, or a legacy session bound by ``register_free_key`` / ``use_api_key``.
