`POST /chat`

## Request

Chat endpoint — proxy to LM Studio.

Receives a user message, appends to session history, sends to LM Studio,
returns the assistant's reply.

Request body:
    message (str): User's message
    session_id (str): Session identifier (UUID)
    history (list): Full conversation history for context

Response:
    reply (str): Assistant's reply
    session_id (str): Same session ID
    sources (list): Sources used (always ["agent-docs/index.md"])
