  - `decision` string, nullable: Moderation decision: allow, block, unchecked
  - `content_checked` boolean, nullable: Whether content was checked by the vision model
  - `code` string, nullable: Error code if blocked or rejected, e.g. IMAGE_REJECTED
  - `message` string, nullable: Message describing the outcome
