`POST /api/v1/mcp/consent/disconnect`

Revoke an assistant's access for THIS account only.

Revokes refresh grants and approved codes, which stops renewal. Tool calls
check the authorization and grant revocation live, so existing access
tokens are rejected too, including on an already-open MCP session.
