Memory Health Monitor — per-user retrieval quality snapshot.

Aggregated every 6h from the trailing 24h window of scored searches.
Helps detect silent quality drops (every search returns 200, but
relevance is degrading). See blog post on Memory Health Monitor

Returns 404 if user has fewer than 5 scored searches in the window
(insufficient signal — no snapshot computed yet).
