For a `decision_without_work` insight, this IS Decision Pulse's
"Kill" — a reason is required (min 3 chars, PRD §5.2) so a dropped
decision is on record with why, not just silently gone, and the
reason is also written into the decision's own history timeline
(record_pulse_event) so DecisionDetail shows it without a second
lookup.
