AI/vote-proposed decisions from any visible session that haven't
yet been promoted into a real decision (PRD §13's "Confirm" strip).
`insights_for_user` already attaches `decisionId` (None = still just
a proposal) to each flattened entry — see db/sessions.py's
`_attach_promoted_decision_ids`.
