`POST /api/records/{record_id}/re-extract`

Re-run AI extraction on an already-uploaded record.

Lets a patient repair a record whose medications/labs were mis-read the
first time (common with handwritten Indian prescriptions) without having to
upload the document again. Re-reads the stored file, rebuilds fhir_data and
re-indexes it for search/chat.
