Rebuild real tool journeys from session_journey events (the ordered per-run
sequences the client flushes on heartbeat). Answers "what do users chain, and
where does it break" for the local fleet — the 92% with no per-call sequence
elsewhere. Break point = the step a run ABORTS on (last step failed or >15s).
