How the machines seen in the window got their relay approval — per platform, counted by
MACHINE (a machine is approved if ANY of its live tokens is), split by how the FIRST approval
was made (lmcp-api#38's approved_via): `tray_button`, `oauth_dialog`, `not_recorded`
(approved before the column, incl. the 2026-09-29 backfill — never read as "dialog"), and
`not_approved`. Test machines and @local-mcp.test are excluded. Bounded: tunnel_tokens only,
a window, grouped — no heartbeat scan.
