First-run activation funnel for machines installed in the window. Splits the
NON-activated (0 successful tool calls) into two churn causes needing different fixes:
  - blocked_at_activation: tried tools but hit a permission wall (TCC/FDA/governance)
    → recoverable; nudge to grant.
  - never_tried: no calls AND no errors → the AI never surfaced/called tools (discovery /
    onboarding gap), not a permission problem.
activated = >=1 SUCCESSFUL call (total_tool_calls counts successes). Validated against
prod (127 installs/14d → 62% activated, 48 never_tried, 0 blocked → discovery is the gap).
