# Record a human-in-the-loop approval event

Append-only log for client-side HITL flows. Lithtrix records the event; it does **not** enforce tool-call interception. When ``tool_id`` is set, the active tool must exist and ``risk_class_declared`` snapshots the tool's registrant-declared ``risk_class``.
