Store one append-only feedback event (helpful / unhelpful / wrong) referencing a prior ``search_id``, ``memory_key``, ``blob_id``, ``parse_id``, or ``browse_id``. **Reference rows are not verified** — ``ref_id`` is not checked for ownership or existence (fast path; feedback is stored only for downstream use). Avoid secrets or PII in ``note`` (max 500 characters).
