Generate-or-refresh the cold-outreach draft.

Pre-2026-05-18 this route lived on Vercel and burned an LLM call on
every operator click. Now cache-first per (workspace, username,
locale) with 30-day TTL; ``refresh=true`` forces a fresh generation.

- `locale` string: Output locale — en / zh-CN / ja
- `refresh` boolean: Force fresh LLM call + cache replace
