Results are **not** persisted — the caller reviews suggestions and calls
bulk-update to save them.

At most ``_BULK_RETRAIN_MAX_BLOCKS`` conditions are processed per request.
Concurrency is bounded by a semaphore to avoid overwhelming the LLM API.
