Let the reviewer fill out the IRA questionnaire themselves. Cancels any
outstanding request and opens a blank response addressed to the reviewer,
which they then fill in and submit through the normal form endpoints
(GET/PUT /forms/response/{id}, POST /forms/response/{id}/submit) — that
submit path already triggers the same scoring as a vendor submission.

A task that skipped the questionnaire at review creation (manual_override,
form_id null) has no form to reuse — this instead creates one from the
review flow's configured IRA template, if any. That form-less path is also
available on an already-completed task, mirroring "send questionnaire"
(reassign): a reviewer who set the assessment manually may change their mind
and fill out the real questionnaire after all, which reopens the task.

A task with an actual form can only be self-completed while still pending —
once it's completed, the assessment is terminal and this is no longer
available for it.
