`GET /v1/admin/source-connections/{id}/items/{itemId}`

## Request

The item, the document it became (or the asset it was stored as and the parts the bridge made of it), the representations the processors extracted, and the facts that cite it with the revision each was read at, its stale mark and its close. Source plane operator surface (docs/roadmap/raw-evidence-sources-2026-09.md) — answers a bare 404 until `SOURCE_PLANE_ENABLED=1`.
