JSON-RPC endpoint — streamable HTTP (MCP 2026-07-28), the only transport.

Stateless: no protocol-level session — authenticate per request and return
the JSON-RPC response in the POST body. The legacy SSE reply channel
(?connection_id=… routing the response onto that client's event stream) was
retired 2026-07-28 and answers 410 Gone.
