Move a workspace onto a custom enterprise plan with bespoke limits.

Payment is handled outside self-serve: either a provider subscription created for
the deal (link it via billing_subscription_id so renewals sync) or an off-platform
invoice (leave it unset — a manual grant renewed by hand).
