Creates a custom agent tool association.

  - `mcp_server_name` string, required: MCP server name
  - `tool_name` string, nullable: Tool name to associate (null for unresolved)

  - `mcp_server_name` string, required: MCP server name
  - `user_id` string, uuid, required: User UUID who owns the association
