Upsert the per-payor portal configuration: credentials plus billing config.

``totp_secret`` is the portal's authenticator (TOTP) base32 seed; the operator
generates the rotating 2FA code from it at login time. ``billing_provider`` and
``service_line`` are the non-secret per-payor defaults merged into every task's params.
