Every stage between "queued" and "hot lead", per account and per campaign.

The one question this answers is WHERE the number died: an account under
the hot-lead floor is failing at exactly one of capture, placement,
audience or copy, and until these stages were measured separately the only
diagnosis available was rereading a dashboard that shows totals.
