Turn a campaign name into a built, review-ready campaign.

Everything past the name already existed and already worked - proposing a
grounded ICP, saving it, filling the pool - it was only ever reachable from
the automatic onboarding run, once, at signup. A customer who wanted a
second campaign had no way to run that sequence again: the only INSERT INTO
v2_campaigns anywhere in the backend was the one-time seed at account
creation, gated to refuse running twice. This is that same sequence, made
reachable on demand.
