Founder-only: release a review-gated case's output to the customer.

Flips PENDING_REVIEW → COMPLETED, records who approved and when, and
publishes the terminal `investigation_completed` event so any open stream
(the customer's live view included) resolves to the report without a
refresh. Loaded without a team filter: founders approve across teams and
require_superuser already gates access.
