TLS issuance gate for the ingress (Caddy on_demand_tls ask).

200 = this hostname is the verified custom domain of an entitled org, issue
a certificate; 404 = refuse. Must stay unauthenticated and cheap, and fail
closed: garbage SNI is rejected before any DB work, errors refuse issuance.
