heartbeat_coverage_pct measures credited heartbeat/session-enforcement time divided by cumulative joined time. in_focus_pct is a deprecated compatibility alias. A session is missed, with opted_out_reason blocking_permissions_missing, when the student's latest explicit permission report at the end of the session was false (not a measure of how long permissions were missing); declined sessions carry opted_out_reason declined.
