The BLANK form for one of our built-in consent documents, as a PDF.

Public by design: it is the text every student of that school is shown
before they decide, with empty lines where the names and signatures go,
and nothing about any person on it. It is what a family prints to sign on
paper, and the same bytes the signed copy is written onto
(`consent_document_pdf`, `parental_consent_service.builtin_form_pdf`).
A version that is not a built-in document is a 404, said plainly.

Sync `def`: the first render of a version takes a few hundred ms; it is
cached per process after that.
