`POST /api/v1/auth/refresh`

Exchange a valid refresh token for a fresh access + refresh pair.

Refresh tokens are single-use (rotated). Browser clients don't need to send
a body - the refresh_token cookie is read automatically.

## Request body

- RefreshTokenRequest: Body for POST /auth/refresh. `refresh_token` is optional - if omitted, the endpoint reads the `refresh_token` cookie (browser flow). API clients pass it in the body.
