One page of the pending-lead index, for the app's "more leads" fast path.

The first page is already in `session.instructions`; this is how the user
hears the rest. Offset paging, the same shape as the MCP `list_prospects`
cursor: the app passes back `next_offset` until it is null. `spoken` is
the page rendered as prose the app can hand straight to the model.
