Live circuit-breaker state for every self-healing dependency.

In-memory only (no DB or external calls) so it stays instant and safe to
poll. `degraded` lists any dependency whose breaker is not CLOSED — when it
is non-empty, that external provider is currently being short-circuited and
served via fallback. The data-level self-heal report lives at
GET /cron/self-heal-status (auth-gated, does DB I/O).
