# List form templates

`GET /api/form-templates`

Lists all form templates with optional pagination.

## Query parameters

- `page` integer, nullable
- `pageSize` integer, nullable

## Response `200`

Form templates retrieved successfully.

- FormTemplateModelWithoutJsonData[]
  - `id` string, nullable — The ID of the form template.
  - `name` string, nullable — The name of the form template.
  - `description` string, nullable — The description of the form template.
  - `isEnabled` boolean — Indicates if the form template is enabled.
  - `createdAt` string, date-time — The date and time when the form template was created.
  - `modifiedAt` string, date-time, nullable — The date and time when the form template was last modified.

## Other responses

- `400` — Invalid input data.
- `401` — Unauthorized
- `403` — Forbidden
