# Get a form template by ID

`GET /api/form-templates/{formTemplateId}`

Gets a form template by its ID.

## Path parameters

- `formTemplateId` integer, required

## Response `200`

Form template retrieved successfully.

- FormTemplateModel — Model of a form template.
  - `id` string, nullable — The ID of the form template.
  - `name` string, nullable — The name of the form template.
  - `description` string, nullable — The description of the form template.
  - `isEnabled` boolean — Indicates if the form template is enabled.
  - `createdAt` string, date-time — The date and time when the form template was created.
  - `modifiedAt` string, date-time, nullable — The date and time when the form template was last modified.
  - `jsonData` string, nullable — The JSON data representing the form template structure.

## Other responses

- `400` — Invalid input data.
- `401` — Unauthorized
- `403` — Forbidden
- `404` — Form template not found.
