Convert an uploaded file to clean Markdown (RAG ingestion building block).

Auto-detects the input format by extension and dispatches to the right
extractor: PDF, DOCX, PPTX, XLSX, CSV, HTML, EPUB, plain text/Markdown, and
legacy/ODF office (DOC/PPT/XLS/ODT/ODS/ODP/RTF via unoserver). The output is
a single .md file with heading-aware structure suited to semantic chunking.

Args:
    file: The document to convert.
    output_filename: Optional custom output name (defaults to input name .md).
    direct_download: If True (default), return the file; else JSON metadata.
