Render one topic hub page: graph-authoritative docs, FTS fallback.

The graph is the authoritative topic index; when it is unavailable the
Turso FTS pool renders the page instead. When the graph is UP but has
nothing for the name, the page 404s (never the fallback — a cleaned
stopword must not resurrect as a phantom hub). One exception, added
2026-10-16 after a ~55 s blip served 404s for 24 real hubs to crawlers:
a name THIS PROCESS served from the graph within the last 15 minutes
serves that recent answer stale, marked `X-Served-From: last-known-good`
and edge-capped at 5 minutes, instead of 404ing. Authority stays with
the graph — a never-seen name still 404s, and a deletion converges
within the TTL.
