`DELETE /api/v1/integrations/health-connect/disconnect`

## Request

Remove the Health Connect integration and everything it imported.

Deleting the category cascades (DB-level ON DELETE CASCADE) to its metrics, their
measurements, and the integration row (user_integrations.category_id) — mirroring
the Oura disconnect. Nothing is written back to Health Connect on the device.
