Live tokens only. The token itself is never returned (only its SHA-256 is stored).

  - `label` string, nullable
  - `created_at` string
  - `last_used_at` string, nullable: null until the extension has made one call
  - `expires_at` string: sliding: +180 days on every authenticated call
