Handle the GitHub user-authorization callback (legacy, non-Nango path).

Exchanges the code for a user-to-server token and checks for installations.
GitHub also appends ``installation_id``/``setup_action`` when the
authorization was triggered by an App install; both are ignored here — the
install itself is bookkept by the webhook, and the install prompt is driven
by ``has_installations`` below.
