Absolutely set patient payment by claim control number (claim-id). Caps at ledger paid + remaining PATIENT_AR; excess is unallocated PATIENT_CASH. paymentTraceId is required; retries return the prior result. 404 if the claim is not found.

  - `paymentAmount` number, required: Absolute paid amount to set
  - `paymentDate` string, date
  - `paymentMethod` string
  - `paymentTraceId` string, required: Required processor transaction id for idempotent retries
  - `notes` string: Required when decreasing paid (refund)

## Response `200`

Payment updated (or idempotent replay)

## Other responses

- `400`: Invalid body
- `401`: Unauthorized
- `404`: Claim not found
- `500`: Server error
