Answer a query from attested state, or refuse (ratified envelope).

Anonymous callers are subject to the public spend guard
(app/agent/public_limits.py); hitting a limit returns a normal
refused-shaped envelope, never an error status. ``?include=receipt``
adds the public receipt fields (never the salt or caller_hash).
