Read one Medicare coverage document from Assyro's MCD copy: an NCD's full text (item/service description, indications and limitations of coverage, cross reference) with its dates, or an LCD, proposed LCD or article listing (contractor, states, status, dates, CMS link; its text and codes are not held). The id is the one a search returned ("ncd-20.9.1", "lcd-33930-v54") or a CMS number ("NCD 20.9.1", "L33930", "A57079"); a number reads its in-effect version and lists the others.
