Data Provider Portal (EHR PRD §4, email+password door) — a standalone,
locked-down page (like /asclepius). Served unauthenticated by design; the page
JS gates on an Asclepius data_partner token and shows its own login → forced
password reset → single upload screen. Static assets load from /static/provider/.
