Self-contained patient code-entry page. Used as the entry point when the
landing app isn't configured (LANDING_URL unset) so patient SMS/email links
always reach a real code-entry experience instead of a bare dashboard URL.
Optional ?hs= pre-fills the (non-sensitive) health-system code.
