Coordinator override of the algorithmic initial tier.

Per PRD §13.13, the override changes `current_tier` *but* leaves the
`initial_tier_was_hard_escalator` flag unchanged so the pre-op re-tier
sticky-hard guard still respects the underlying clinical condition.
