This evaluator's walk through one chart (§4 Phase 5 — trajectory mode).

The session view: how many decision points this chart carries, how far this
physician has come, and which point they may open next. Per-evaluator on
purpose — two physicians walking the same chart are two independent
trajectories that happen to share a case set, and a shared "7 of 13" would be a
lie to both of them.

Returns METADATA ONLY. No case content, no prompts, no outcomes: the sequence
gate exists precisely so a physician cannot read ahead, and a session view that
rendered every point's chart would be that leak wearing a progress bar. Each
point is opened through the ordinary by-ID path, which re-checks the gate.
