Correct the handful of fields that are the physician's own to correct.

- ProfileUpdate: PATCH /me/profile — the fields a physician may correct about themselves. Deliberately short, and deliberately not the credential fields: those were checked against a registry or attested to, and letting someone edit them after approval would make the check meaningless. Every field is optional because a partial update is the normal case; ``None`` leaves a column alone, while an empty string clears it.
