Ingested case → gradable V4 task (PRD §9): attach the clinical question,
render the case prompt, generate candidates CONDITIONED ON THE REAL CASE,
gate (hardness + real-variant case judge — no ground-truth dimension: the
specialist is the answer key), insert as a partner_ehr task. Needs an LLM.

``?dry_run=true`` (or ``{"dry_run": true}``) runs every gate and returns the
sample WITHOUT committing anything — see the block below. The query parameter
exists because this is a thing you reach for from a terminal mid-debug, and
it only ever turns the dry run ON: a query string cannot force a commit that
the body asked to be a dry run.
