## Request body

- GenerateRealCasesRequest: Build V4 tasks from ONE ingested real chart (Real-Case Generation PRD §5). ``dry_run`` is not optional in spirit and defaults to True here: an admin generating seven cases from a chart must be able to read every proposed question, index event, tag and difficulty band BEFORE any of it reaches a physician. Generating a wrong question at scale is how you burn physician goodwill, and the plan is cheap — the dry run makes no model call at all unless ``derive_questions`` asks for one.
