Load the ratified GOLD multimodal cases for a specialty (Two-Model PRD
Workstream C — the "load gold" half of the load-vs-generate split).

Unlike ``POST /generation/{specialty}`` (which synthesizes NOVEL cases via the
LLM and needs a key), this inserts the hand-authored, clinician-ratified seed
cases — real labs + EHR + an authored A/B pair — with NO LLM required. Idempotent
(stable ``gold-<case_id>`` task ids are skipped if already present).
