The ENV annotation queue (PRD §7.2). Gated on ``portal_version == 'env'``.

``allow_legacy``: a client cached before the rename still sends ``'v5'``. It is
accepted for one release rather than 400ing a physician mid-annotation over a
string their page will stop sending the moment it reloads. Nothing is STORED
from it — the response echoes ``ENV_PORTAL_VERSION``.
